1. GET AN APPLICATION NUMBER.

2. FILL OUT THE APPLICATION FORM.

5. SUBMIT YOUR ADMISSION CREDENTIALS.

ADMISSION CREDENTIALS FOR FRESHMEN:

1. Form 138 / High Card
2. Good Moral Certificate
3. Clear Copy PSA Birth certificate
4. 2pcs of 2×2 ID picture with white background and with name tag

You may send the original copy of your admission credentials via LBC:

6. RECEIVE THE ELECTRONIC COPY OF YOUR REGISTRATION FORM

You will receive an electronic / pdf copy of your official registration form upon receiving your credentials.

Original Copy of Registration Form will be given upon resumption of face-to-face transactions.
