The Office of Student Affairs and Services opens its doors to accepting applications for PhilHealth membership and updating of records. PhilHealth membership is one of the internship requirements under the Palaestra Consortio Office.
Interested applicants shall be at least 21 years old and above and must submit the following:
* PSA birth certificate (photocopy)
* Formal ID picture (1x1 size, 2 pcs)
* Application Form (available at OSAS)
* Php 500.00 (based on 2024 rate)
Applicants may submit requirements every Monday to Thursday, 8:00 a.m. to 5:00 p.m. at OSAS. Release of Member Data Record and ID will be a week after submission to OSAS.
